Labor Standards Interview

Labor Standards Interview (Standard Form 1445, Rev. 12-96) is a Davis-Bacon compliance form used to document on-site interviews with construction workers on federally funded projects. The purpose of the form is to verify that employees are being properly classified and paid the prevailing wage rates required under the Davis-Bacon Act and related labor standards.

The form records employee information, employer and contractor details, work classification, wage rate, hours worked, overtime practices, fringe benefit payments, and payroll deductions. It also allows the interviewer to document the work being performed at the time of the interview and compare the employee's responses to certified payroll records to identify any discrepancies or potential labor standards violations.

An online form is available at Labor Standards Interview Form.

For reference, the Labor Standards Interview Form (SF 1445) is provided; however, completion and administration of this form are the responsibility of the contractor and/or project owner.
